This program will use a variety of products/techniques and may include activities which could be considered harmful. In order for your teen to participate, a Release and Hold Harmless form must be signed by a parent or legal guardian prior to the start of the program.

Participants may be photographed/recorded and/or surveyed to capture the program's effectiveness. If you do not want your teen photographed/recorded and/or surveyed, please fill out the Audio/Visual Recordings Opt-Out form and/or the Surveys Opt-Out form.
